Telehealth platform gives members living with congestive heart failure or uncontrolled diabetes the ability to manage their condition from home
Prestige Health Choice, a Medicaid managed care plan serving 55 of Florida’s 67 counties and a joint venture between AmeriHealth Caritas (Opens in a new window) and Florida Blue (Opens in a new window), has expanded the footprint of its remote biometric monitoring program into Brevard, Manatee, Orange, Osceola, Sarasota, Sumter and Wakulla counties, giving more health plan members living with congestive heart failure or uncontrolled diabetes mellitus a new way to monitor their health status.
The program, a collaboration with Vheda Health, launched in Pinellas, Hillsborough, Pasco, Polk and Highlands counties in October 2017. Vheda Health sends participants a cellphone, weight scale, blood pressure monitoring cuff, and pulse oximeter to measure oxygen values. Pilot participants diagnosed with diabetes mellitus also receive a blood glucose meter for daily glucose checks.
